The programs help ensure that patients understand their disease process so they can have safety and disease management at home and ultimately decrease their hospitalizations.
“Each program is very specific to that disease and it provides the education needed to manage their disease,” Angels Care Home Health Branch Manager Shannon Hiegel said.
For instance, if a patient learns he or she has congestive heart disease, Angels Care Home Health will teach the patient and their family the in-depth definition of the disease, the specific medications the patient is taking for their diagnosis.
“With congestive heart failure it is important for patients to measure their weight daily,” Hiegel said. “We will show them to how to keep a weight log and do everything possible to keep them home and out of the hospital.
“A program such as the one for congestive heart disease will help the patient know the levels for when they should call us, the doctor or at the stage it is serious enough to dial 9-1-1,” Hiegel added.
Angels Care Home Health offers these disease management programs in areas such as:
• Diabetes. The program includes teaching the patient to self-manage his or her blood glucose levels, diet, exercise, administering insulin injections, wound evaluation and care as well as information regarding their medication regimen.
• Chronic obstructive pulmonary disease. Its program includes education on energy conservation, medications, reducing the risk of infection, activity and exercise and a patient self-care workbook.
• Congestive heart disease. Its program includes a focus on the patients self-management through education of symptoms, daily weighing, recording and monitoring, activity and exercise and dietary recommendations.
• Alzheimer’s disease. Its program includes a teaching focus for the caregiver to help them understand and deal with the disease process, which will help their care-giving skills.
• Stroke/TBI (Traumatic Brain Injury). Its program is designed for both the patient and the caregiver to help the patient rehabilitate at home and provide the caregiver education.
• Falls prevention. This program will help decrease the chance of future falls and hospitalizations.
• Hypertension. Its program focuses on self-management through education on symptoms, blood pressure monitoring, medications, diet, prevention strategies and a comprehensive cardiac assessment.
• Parkinson’s Disease. Its program focuses on self-management through education, symptoms management and medication, as well as a depression assessment.
“The biggest thing is giving the patients a sense of control over their lives and allowing them to maintain their independence,” Hiegel said.
